The combination involves metformin, clomiphene citrate, and human chorionic gonadotropin (hCG). **Therapeutic Use and Mechanism** Metformin and clomiphene citrate work through different mechanisms to improve fertility outcomes in women with PCOS: * **Metformin** helps by: * Improving insulin resistance, which is common in PCOS patients * Reducing hyperinsulinemia, which contributes to the hormonal imbalances in PCOS * Increasing sensitivity to clomiphene citrate in women who are resistant to clomiphene alone * Normalizing menstrual cycles * **Clomiphene citrate** works by: * Inducing ovulation in women who do not ovulate on their own * Acting as a selective estrogen receptor modulator **Human chorionic gonadotropin (hCG)** is typically added to fertility treatments to trigger ovulation after follicular development has been achieved with medications like clomiphene. **Clinical Efficacy** The combination of metformin and clomiphene has demonstrated several benefits, with hCG added to trigger ovulation: * Higher ovulatory rates compared to clomiphene alone, with statistical significance at doses of 100 and 150 mg of clomiphene. * Improved pregnancy rates compared to clomiphene alone, though some studies found this difference was not statistically significant. * Particularly effective in clomiphene-resistant PCOS patients, with one study showing a 75% ovulation rate and 55% pregnancy rate with the combination versus 27% ovulation and 7% pregnancy rates with clomiphene alone. * Increased sensitivity to clomiphene, allowing women who previously failed to respond to clomiphene to achieve ovulation. When lifestyle modifications are added to the metformin and clomiphene combination, additional benefits include significant reduction in body weight and BMI, improvements in reproductive endocrine and lipid metabolism parameters, decreased ovarian volumes, and higher menstrual recovery, ovulation, and pregnancy rates. **Treatment Approach** For women with PCOS who do not achieve pregnancy with metformin alone, adding clomiphene is considered a reasonable option. This combination approach, followed by an hCG trigger, may help patients avoid more expensive and invasive treatments such as injectable FSH medications or in vitro fertilization. Success rates depend on various factors including female age, weight or BMI, sperm parameters, and other fertility issues. For women under 37 with normal sperm parameters who achieve ovulation, pregnancy success rates are approximately 10-15% per month with timed intercourse and 15-20% per month with intrauterine insemination (IUI). If this combination fails to result in ovulation and pregnancy, more aggressive treatments may be considered.
