This is a fixed-dose combination of three oral antidiabetic agents—metformin, acarbose, and pioglitazone—used for the management of type 2 diabetes mellitus. Each component has a distinct mechanism of action: - **Metformin** is a biguanide that decreases hepatic glucose production, reduces intestinal absorption of glucose, and improves insulin sensitivity. - **Acarbose** is an alpha-glucosidase inhibitor that delays carbohydrate digestion and absorption in the intestine, thereby reducing postprandial blood glucose spikes. - **Pioglitazone** is a thiazolidinedione (TZD) that enhances insulin sensitivity in adipose tissue, muscle, and the liver by activating peroxisome proliferator-activated receptor gamma (PPARγ). The combination aims to provide complementary glycemic control by targeting different pathophysiological aspects of type 2 diabetes. This triple therapy may be considered for patients inadequately controlled on dual or monotherapy with these agents.
