Overview

The combination of metformin and clomiphene citrate is a therapeutic approach primarily used for treating infertility in women with polycystic ovary syndrome (PCOS). This combination therapy has been extensively studied for its efficacy in improving ovulation and pregnancy rates compared to either medication used alone. ## Mechanism of Action Metformin works as an insulin-sensitizing agent that improves insulin resistance, which is common in PCOS patients. By reducing insulin resistance, metformin helps restore normal hormonal balance and can improve ovarian function[1][4]. Clomiphene citrate (CC) functions as an anti-estrogen that competes for estrogen receptors centrally, releasing the hypothalamus and pituitary from negative feedback, which stimulates ovulation[3]. When used together, these medications appear to have complementary effects that enhance ovulation rates in women with PCOS. ## Clinical Efficacy Research shows that the metformin-clomiphene combination results in significantly higher ovulation rates compared to clomiphene alone (p = 0.0016)[1]. Studies have demonstrated pregnancy rates of approximately 24% with the combination therapy versus 8% with clomiphene alone[1]. The combination therapy has also shown effectiveness in women who failed to conceive with clomiphene alone (CC failures), suggesting that metformin increases sensitivity to clomiphene[1]. The mean duration for conception with the combination therapy is approximately 3.8 months, compared to 6 months with clomiphene alone[1]. ## Comparative Effectiveness There are some contradictions in the literature regarding whether metformin, clomiphene, or their combination is superior for improving pregnancy rates. Some studies suggest the combination is better than clomiphene alone[4], while others have found no additional benefit from adding metformin to clomiphene[4]. A large randomized trial of over 600 women found that clomiphene was superior to metformin in achieving live births in infertile women with PCOS (22.5% vs. 7.2%), with the combination therapy showing a rate of 26.8%[2]. However, this difference between clomiphene alone and the combination therapy was not statistically significant (p=0.31)[2]. ## Side Effects and Considerations The combination therapy has a different side effect profile compared to either medication alone. Metformin is associated with more gastrointestinal side effects but fewer vasomotor and ovulatory symptoms compared to clomiphene[2]. Multiple pregnancy is a potential complication with clomiphene-containing regimens, with rates of approximately 6% for clomiphene alone and 3.1% for the combination therapy[2]. ## Current Status Metformin and clomiphene, alone or in combination, are considered first-line agents for ovulation induction and treatment of infertility in patients with PCOS[4]. The choice between these treatment options may depend on individual patient characteristics, including body mass index, insulin resistance status, and previous treatment responses.
