Metformin + fiber is an investigational combination therapy being evaluated by researchers at the University of Alberta for the treatment of severe obesity and insulin resistance in adolescents. Metformin is a biguanide pharmaceutical that primarily acts by activating AMP-activated protein kinase (AMPK), which leads to a reduction in hepatic glucose production and an increase in peripheral insulin sensitivity. The fiber component in this specific study is a 35 g mixture consisting of oligofructose, resistant maltodextrin, acacia gum, and PGX (a proprietary polysaccharide complex). The combination is hypothesized to have a synergistic effect, combining metformin's metabolic regulation with fiber's ability to modulate the gut microbiome, slow gastric emptying, and promote satiety. This dual approach aims to improve glucose metabolism and reduce body mass index (BMI) and systemic inflammation more effectively than either agent alone.
