metformin + Gelesis100 refers to the co-administration of the biguanide antihyperglycemic agent metformin with Gelesis100 (marketed as Plenity), a non-systemic, superabsorbent oral hydrogel device. Developed by Gelesis, Inc., this combination was evaluated in clinical pharmacokinetic studies (such as NCT02311101) to determine if the mechanical presence of the hydrogel in the gastrointestinal tract affects the absorption and systemic exposure of metformin in overweight or obese individuals. Metformin is a first-line therapy for type 2 diabetes mellitus that works primarily by inhibiting mitochondrial complex I and activating AMP-activated protein kinase (AMPK), leading to reduced hepatic gluconeogenesis and improved insulin sensitivity. Gelesis100 is a medical device composed of cross-linked cellulose and citric acid particles that, when ingested with water, expand to occupy space in the stomach and small intestine to promote satiety and weight loss. While Gelesis100 is FDA-cleared for weight management in adults with a BMI of 25–40 kg/m², the company also developed Gelesis200, a related hydrogel specifically optimized for glycemic control and weight loss in patients with type 2 diabetes.
