Overview

This is a combination therapy consisting of three agents commonly used in the management of type 2 diabetes mellitus: - **Metformin** is a biguanide that primarily decreases hepatic glucose production (gluconeogenesis), reduces intestinal absorption of glucose, and increases insulin sensitivity. - **Glimepiride** is a sulfonylurea that stimulates pancreatic beta cells to increase endogenous insulin secretion. - **Insulin** refers to exogenous human or analog insulins administered by injection to directly lower blood glucose by promoting cellular uptake and storage. The combination targets multiple pathophysiological defects in type 2 diabetes—insulin resistance (metformin), impaired insulin secretion (glimepiride), and absolute or relative deficiency of endogenous insulin (exogenous insulin). This regimen can be used when monotherapy or dual therapy fails to achieve glycemic control. Studies show that adding glimepiride to ongoing metformin and insulin therapy can further reduce HbA1c levels and/or decrease the required dose of exogenous insulin[6][8][10]. The triple combination may improve overall glycemic control but requires careful monitoring for hypoglycemia.
