Overview

This combination therapy consists of the biguanide metformin and marine-derived omega-3 polyunsaturated fatty acids, specifically eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). It is being investigated in a pilot clinical trial (NCT02278965) led by Katherine D. Crew at Columbia University for its feasibility and biomarker modulation in women with a history of early-stage breast cancer. Metformin acts primarily by activating AMP-activated protein kinase (AMPK), which regulates metabolic pathways and inhibits hepatic glucose production. Omega-3 fatty acids modulate inflammatory pathways, act as ligands for GPR120 (FFAR4), and inhibit pro-inflammatory eicosanoid synthesis. The study aims to evaluate changes in breast tissue, blood biomarkers, and mammographic density over a one-year period to assess the potential for breast cancer prevention and survivorship support.
