Metformin + pemetrexed is a combination of two small molecule drugs used primarily in research and clinical trials for the treatment of advanced non-small cell lung cancer (NSCLC), particularly lung adenocarcinoma. Metformin is an oral antihyperglycemic agent commonly used to treat type 2 diabetes, while pemetrexed is a folate antimetabolite chemotherapy agent approved for NSCLC and mesothelioma. The combination has been investigated due to preclinical evidence suggesting synergistic antiproliferative, proapoptotic, and antiangiogenic effects on tumor cells. Mechanistically, metformin activates AMP-activated protein kinase (AMPK), leading to inhibition of the mTOR pathway and reduced tumor cell proliferation; it may also sensitize cancer cells to chemotherapy-induced apoptosis. Pemetrexed inhibits multiple folate-dependent enzymes involved in nucleotide synthesis, thereby blocking DNA replication and inducing cell death in rapidly dividing cells. Clinical studies have shown that this combination can improve survival outcomes as first-line therapy for diabetic patients with advanced lung adenocarcinoma compared to standard regimens[1][2][5]. Preclinical models demonstrate additive or synergistic effects on reducing tumor growth, angiogenesis (via VEGF/VEGFA downregulation), and increasing apoptosis[3][4].
