Metformin + pioglitazone + SGLT2 inhibitor

Metformin + pioglitazone + SGLT2 inhibitor is a triple oral combination therapy used in the management of type 2 diabetes mellitus. This regimen combines three agents with complementary mechanisms of action to improve glycemic control and insulin sensitivity. Metformin, a biguanide, lowers blood glucose by increasing peripheral glucose uptake and decreasing hepatic glucose production, likely via inhibition of mitochondrial enzymes[6]. Pioglitazone is a thiazolidinedione (TZD) that acts as an agonist at peroxisome proliferator-activated receptor gamma (PPARγ), promoting transcriptional changes that enhance insulin sensitivity in adipose tissue and other organs[1][5]. SGLT2 inhibitors (such as dapagliflozin or empagliflozin) block sodium–glucose cotransporter 2 in the proximal renal tubules, reducing renal glucose reabsorption and increasing urinary glucose excretion[5][6]. Clinical studies have shown that this triple combination can significantly improve glycemic parameters, reduce insulin resistance, favorably affect lipid profiles, lower body weight and blood pressure, and has a low risk of hypoglycemia; however, there may be an increased risk of genital mycotic infections[2][4][5].
