Metformin 1% gel is an investigational local drug delivery system used as an adjunct to scaling and root planing (SRP) for the treatment of periodontal defects, specifically Class II furcation defects, in patients with chronic periodontitis. Developed by researchers at the Government Dental College and Research Institute in Bangalore, this biodegradable gel formulation is designed for subgingival application to provide controlled release of the active ingredient. Metformin, a biguanide small molecule traditionally used for type 2 diabetes, is utilized in this context for its pleiotropic bone-sparing properties. It acts by stimulating osteoblast differentiation and inhibiting osteoclastogenesis, thereby promoting alveolar bone regeneration and the repair of intrabony defects when delivered locally to the site of infection.
