Metformin + testosterone undecanoate is a combination therapy used primarily for men with type 2 diabetes mellitus (T2DM) who also have hypogonadism. This combination addresses both glycemic control and testosterone deficiency simultaneously. Metformin works by decreasing hepatic glucose production, decreasing intestinal absorption of glucose, and increasing insulin sensitivity through peripheral glucose uptake and utilization[3]. It inhibits mitochondrial complex I, preventing ATP production and activating AMP-activated protein kinase (AMPK), which plays an important role in glucose metabolism[3]. Testosterone undecanoate is an ester prodrug of testosterone with a mid-chain fatty acid at the carbon 17β position. It was developed via fatty acid esterification of testosterone to enable oral administration[5]. Testosterone is responsible for normal growth and development of male sex organs and maintenance of secondary sex characteristics. Testosterone replacement therapy aims to restore testosterone levels in hypogonadal men, improving symptoms like decreased libido, erectile dysfunction, and loss of muscle and bone mass[5]. Studies have shown that the combination of metformin and testosterone can provide additional benefits compared to either medication alone in men with hypogonadism and impaired glucose tolerance or T2DM[2][7]. However, it's important to note that metformin may actually decrease testosterone levels in men with T2DM, potentially counteracting the testosterone elevation that accompanies improved blood glucose control[1][6].
