This combination has been studied in several contexts: - In polycystic ovary syndrome (PCOS), the addition of calcium-vitamin D to metformin therapy has shown improved outcomes in menstrual regularity, follicular maturation, and ovulation compared to individual treatments[6][8]. - In nonalcoholic fatty liver disease (NAFLD) models, co-supplementation with metformin plus calcium-vitamin D3 reduced hepatic steatosis more than either alone but did not show marked additional benefits over monotherapy[1]. - Preclinical studies suggest synergistic effects on osteogenic differentiation under high-glucose conditions[5] and enhanced chemopreventive activity against early colon neoplasia through AMPK/mTOR pathway modulation[2]. The mechanism of action involves: - Metformin activating AMP-activated protein kinase (AMPK), reducing gluconeogenesis. - Vitamin D binding to the vitamin D receptor (VDR), influencing gene transcription related to cell growth/differentiation. - Calcium supporting bone mineralization and acting as a secondary messenger in cellular signaling.
