Overview

Metformin delayed release (Metformin DR) is a formulation of the antihyperglycemic agent metformin designed to maximize gut-based mechanisms of action by targeting drug release to the ileum. Unlike immediate- or extended-release formulations, Metformin DR is engineered to bypass major sites of absorption in the upper gastrointestinal tract and instead releases at pH 6.5, typically found in the lower jejunum and ileum. This results in approximately 50% lower plasma exposure compared with identical daily doses of immediate- or extended-release metformin but maintains glycemic efficacy due to enhanced local effects on gut hormones such as glucagon-like peptide-1 (GLP-1) and peptide YY. The primary indication for Metformin DR is type 2 diabetes mellitus, where it improves glycemic control as an adjunct to diet and exercise[6].
