Overview

Metformin eicosapentaenoate is a **combination drug** consisting of **metformin** (a biguanide antihyperglycemic agent, widely used in type 2 diabetes) and **eicosapentaenoate** (the salt form of eicosapentaenoic acid, an omega-3 fatty acid). The rationale for this combination is to address both **hyperglycemia** and **hypertriglyceridemia**, conditions commonly co-occurring in type 2 diabetes. Metformin lowers hepatic glucose production and increases insulin sensitivity primarily via **AMP-activated protein kinase (AMPK) activation**. Eicosapentaenoic acid exerts anti-inflammatory effects and reduces triglyceride levels, likely through **modulation of lipid metabolism and inhibition of hepatic lipogenesis**. The combined product was hypothesized to provide improved gastrointestinal tolerability and greater bioavailability compared to separate co-administration, but available data indicate little pharmacokinetic advantage for the combination over separate administration.
