Metgluco-GP2 is a fixed-dose combination (FDC) anti-diabetic medication marketed by Makshi Healthcare, primarily indicated for the management of type 2 diabetes mellitus. It combines three distinct classes of oral hypoglycemic agents: metformin (a biguanide), glimepiride (a second-generation sulfonylurea), and pioglitazone (a thiazolidinedione). Metformin works by inhibiting mitochondrial complex I, which reduces hepatic glucose production and improves peripheral insulin sensitivity. Glimepiride acts as an antagonist at the sulfonylurea receptor 1 (SUR1) on pancreatic beta cells, thereby stimulating insulin secretion. Pioglitazone serves as a potent and selective agonist for the nuclear receptor peroxisome proliferator-activated receptor-gamma (PPAR-γ), which enhances insulin sensitivity in muscle and adipose tissue by regulating the transcription of insulin-responsive genes. This triple therapy is typically employed when glycemic control is inadequate despite treatment with dual therapy.
