Methadyl acetate is a synthetic opioid analgesic primarily used in the treatment of narcotic dependence. It acts as a long-onset, long-duration narcotic that decreases opioid use by preventing withdrawal symptoms and mimicking some effects of opioids. Methadyl acetate is a racemic mixture of alphacetylmethadol and betacetylmethadol, each with two optical isomers, making four possible stereoisomers in total. Its mechanism of action involves agonism at the mu-type opioid receptor, similar to methadone; this leads to reduced pain perception by decreasing nerve conduction and neurotransmitter release in the central nervous system. In the United States, it is classified as a Schedule I controlled substance due to its high abuse potential and lack of accepted medical use[1][2][3][6].
