Overview

Methenamine mandelate is a small-molecule urinary antibacterial and antiseptic consisting of a 1:1 organic salt of methenamine and mandelic acid. After oral absorption and renal excretion, it becomes active in acidic urine: methenamine hydrolyzes to bactericidal formaldehyde, while mandelic acid contributes antibacterial activity and urine acidification. Its antibacterial effect is nonspecific and is used to suppress or eliminate bacteriuria associated with cystitis, pyelonephritis, chronic urinary tract infection, and infected residual urine in patients with neurologic bladder dysfunction. Activity depends on maintaining acidic urine, with maximal effectiveness at urinary pH 5.5 or lower.
