The RTOG 0227 regimen is a multi-modality treatment protocol developed by the Radiation Therapy Oncology Group (now part of NRG Oncology) for the treatment of primary central nervous system lymphoma (PCNSL). The regimen consists of an induction phase utilizing high-dose methotrexate (a folate antimetabolite), rituximab (a CD20-targeting monoclonal antibody), and temozolomide (an oral alkylating agent). This induction is followed by hyperfractionated whole-brain radiotherapy (hWBRT) and subsequent maintenance therapy with temozolomide. The combination is specifically designed to utilize agents capable of crossing the blood-brain barrier to achieve intracranial disease control in diffuse large B-cell lymphomas of the CNS. Clinical trial results demonstrated that this integrated approach achieved significant improvements in overall and progression-free survival compared to historical RTOG trials.
