Overview

MVP-R is a chemoimmunotherapy regimen consisting of methotrexate, vinblastine, procarbazine, and rituximab, primarily indicated for the treatment of primary central nervous system lymphoma (PCNSL). The regimen is designed to provide multi-targeted cytotoxic activity against B-cell malignancies within the CNS. Methotrexate, administered at high doses, serves as the backbone of the therapy due to its capacity to penetrate the blood-brain barrier and inhibit dihydrofolate reductase, thereby disrupting DNA synthesis. Rituximab is a monoclonal antibody that targets the CD20 antigen on the surface of B-lymphocytes, inducing cell death through antibody-dependent cellular cytotoxicity and complement-mediated lysis. Vinblastine, a vinca alkaloid, inhibits microtubule assembly, while procarbazine acts as an alkylating agent that damages DNA. This combination is frequently utilized in elderly patients or those who cannot tolerate more intensive induction protocols, offering a balance between efficacy and toxicity.
