This is a combination chemotherapy regimen consisting of three drugs—methotrexate, prednisone, and vincristine. Each drug has a distinct mechanism of action: - Methotrexate is an antimetabolite that inhibits dihydrofolate reductase, interfering with DNA synthesis and cell replication. - Prednisone is a synthetic glucocorticoid corticosteroid that exerts anti-inflammatory and immunosuppressive effects by modulating gene expression through the glucocorticoid receptor. - Vincristine is a vinca alkaloid that disrupts microtubule formation in mitotic spindle fibers, inhibiting cell division. This combination has been used in various protocols for the treatment of acute lymphoblastic leukemia (ALL) and other hematologic malignancies. It may be part of multi-agent regimens such as POMP (prednisolone, vincristine, methotrexate, 6-mercaptopurine) or VAMP (vincristine sulfate, doxorubicin hydrochloride [Adriamycin], methotrexate, prednisone), but can also be administered as its own combination[2][3]. The rationale for combining these agents lies in their complementary mechanisms to maximize cancer cell kill while minimizing overlapping toxicities.
