Methylene blue + amodiaquine (MB-AQ) is a non-artemisinin-based combination therapy for the treatment of uncomplicated falciparum malaria. Methylene blue, the first synthetic antimalarial drug ever used, works through multiple mechanisms including inhibiting *Plasmodium falciparum* glutathione reductase, inhibiting heme polymerization within the parasite's food vacuole, and preventing methemoglobinemia in clinical malaria. Amodiaquine has remained effective in many sub-Saharan African countries despite emerging chloroquine resistance. The combination has shown excellent efficacy with cure rates of up to 95% by day 28 after PCR-based correction for recrudescence in clinical trials conducted in Burkina Faso. It has been evaluated as an alternative to artemisinin-based combination therapies (ACTs) and has shown superior efficacy compared to MB-AS in clinical trials.
