Overview

Methylfentanyl refers to a group of synthetic opioid analgesics structurally related to fentanyl. The most well-known isomers are 3-methylfentanyl and alpha-methylfentanyl. These compounds act as potent agonists at the mu-opioid receptor and exhibit pharmacological effects similar to fentanyl but with significantly increased potency—3-methylfentanyl is estimated to be between 400–6000 times more potent than morphine. Methyl group substitution on the piperidine ring increases binding affinity for opioid receptors and can alter both potency and duration of action depending on the isomer. These drugs have no accepted medical use in most countries and are associated with high risk of fatal overdose due to respiratory depression. They have been encountered primarily as illicit substances mixed with or sold as heroin ("China White") or other opioids[1][6][8].
