Overview

This is a combination therapy consisting of four pharmacological agents commonly used in the management of attention-deficit/hyperactivity disorder (ADHD): methylphenidate, amphetamine, atomoxetine, and guanfacine. Each component has a distinct mechanism of action: - **Methylphenidate** is a central nervous system stimulant that primarily inhibits the reuptake of dopamine and norepinephrine. - **Amphetamine** is also a stimulant that increases synaptic concentrations of dopamine and norepinephrine by promoting their release. - **Atomoxetine** is a selective norepinephrine reuptake inhibitor (NRI), enhancing noradrenergic transmission without direct dopaminergic stimulation. - **Guanfacine** is an alpha2A adrenergic receptor agonist that modulates prefrontal cortical activity to improve attention and impulse control. Combination regimens involving two or more of these agents are sometimes used off-label for patients with ADHD who have an inadequate response to monotherapy. Evidence suggests such combinations may provide additive or synergistic benefits on symptom control, particularly when stimulants alone are insufficient or not tolerated at higher doses[1][2][3][4]. However, there are no approved fixed-dose products containing all four ingredients together; use in this manner would be highly experimental and not standard clinical practice. Safety data for dual or triple combinations exist but evidence for quadruple therapy remains extremely limited.
