Methylprednisolone + dihydroxypropyltheophylline is a combination therapy being investigated for its potential to optimize respiratory dynamics and mitigate lung injury during one-lung ventilation (OLV) in patients undergoing thoracoscopic lobectomy. The combination consists of methylprednisolone, a synthetic glucocorticoid with potent anti-inflammatory and immunosuppressive properties, and dihydroxypropyltheophylline (also known as dyphylline), a xanthine derivative that acts as a bronchodilator. Methylprednisolone functions by binding to and activating the glucocorticoid receptor, leading to the suppression of inflammatory cytokines and the stabilization of lysosomal membranes. Dihydroxypropyltheophylline primarily acts through the non-selective inhibition of phosphodiesterase (PDE) enzymes, which increases intracellular cyclic adenosine monophosphate (cAMP) levels, resulting in bronchial smooth muscle relaxation and potentially reduced inflammatory mediator release. This combination is being studied by Qianfoshan Hospital to improve perioperative outcomes in thoracic surgery.
