This combination therapy consists of pulse intravenous methylprednisolone (IVMP) and human intravenous immunoglobulin (IVIG). It is being investigated by the University of São Paulo as an optimized induction regimen for patients with idiopathic inflammatory myopathies (IIM), including dermatomyositis and polymyositis. Methylprednisolone is a synthetic glucocorticoid that provides rapid anti-inflammatory and immunosuppressive effects by binding to the glucocorticoid receptor. IVIG is a biologic product composed of polyvalent IgG purified from human plasma; it modulates the immune system through multiple pathways, such as neutralizing pathogenic autoantibodies, inhibiting the complement cascade, and interacting with Fc receptors on immune cells. The protocol aims to achieve faster clinical remission and reduce the long-term requirement for oral corticosteroids in patients with newly diagnosed inflammatory myopathies.
