Metiblo (methylthioninium chloride, commonly known as methylene blue) is an intravenous antidote and diagnostic dye. It is traditionally indicated for the treatment of drug- or chemical-induced methemoglobinemia and used as a visualization agent in diagnostic and surgical procedures. In the context of ophthalmology, methylthioninium chloride is being evaluated in a Phase 2 clinical trial led by Dr. Omer Bialer at Rabin Medical Center in Israel for the treatment of acute nonarteritic anterior ischemic optic neuropathy (NAION). In this indication, it acts as a nitric oxide synthase (NOS) inhibitor and guanylate cyclase inhibitor, aiming to halt the acute ischemic attack, reduce neurotoxicity, and improve visual outcomes when administered as a one-time intravenous infusion within 48 hours of symptom onset.
