Overview

Metildigoxin is a semisynthetic digitalis glycoside closely related to digoxin, differing by the presence of a methyl group on the digitoxose residue. It acts as a cardiac glycoside and is used in the treatment of acute and chronic heart failure as well as certain types of arrhythmias, particularly atrial arrhythmias. Metildigoxin increases myocardial contractility (positive inotropic effect) and decreases heart rate (negative chronotropic effect), primarily by inhibiting the sodium/potassium-transporting ATPase pump in cardiac myocytes. This inhibition leads to increased intracellular calcium concentrations, enhancing cardiac contractility. Compared to digoxin, metildigoxin has a more rapid onset of action and is metabolized in the liver to digoxin[1][2][4][5][6][8].
