Overview

Metoclopramide is a small molecule pharmaceutical drug classified as a prokinetic and antiemetic agent. It is primarily used to treat gastroesophageal reflux disease (GERD) that does not respond to other treatments, diabetic gastroparesis (delayed stomach emptying), and for the prevention of nausea and vomiting associated with chemotherapy, surgery, or certain medical procedures. Metoclopramide works by increasing muscle contractions in the upper digestive tract, thereby speeding gastric emptying and improving gastrointestinal motility. Its mechanism of action involves antagonism of dopamine D2 receptors (both centrally in the chemoreceptor trigger zone and peripherally in the gut), agonism at serotonin 5-HT4 receptors, weak antagonism at serotonin 5-HT3 receptors, and enhancement of acetylcholine release via muscarinic receptor modulation. These actions result in increased lower esophageal sphincter tone, accelerated gastric transit, and antiemetic effects[1][2][4][6].
