A combination of three antibiotics—metronidazole (a nitroimidazole compound), ciprofloxacin (a synthetic fluoroquinolone), and minocycline (a semisynthetic tetracycline derivative)—used primarily as an intracanal medicament in endodontics. This mixture is known as Triple Antibiotic Paste (TAP) or 3-mix paste. It was developed to address the polymicrobial nature of root canal infections that cannot be effectively treated by a single antibiotic. The combination provides broad-spectrum antibacterial activity against gram-positive bacteria, gram-negative bacteria, and anaerobes. Metronidazole targets anaerobic bacteria and protozoa; ciprofloxacin is bactericidal with high activity against gram-negative organisms; minocycline is bacteriostatic with a broad spectrum including both gram-positive and some gram-negative organisms. TAP is used for disinfection in regenerative endodontic procedures such as pulp revascularization and lesion sterilization/tissue repair therapy[1][2][3][4][6].
