Mevalonolactone is a small molecule and the lactone form of mevalonic acid, a critical intermediate in the mevalonate pathway responsible for the biosynthesis of cholesterol and non-sterol isoprenoids. It is being investigated as a therapeutic for HMG-CoA reductase (HMGCR) deficiency-related myopathies, such as limb-girdle muscular dystrophy, and statin-induced myopathy. In these conditions, mevalonolactone acts by bypassing the enzymatic block at HMGCR to restore downstream isoprenoid production. Additionally, mevalonolactone is utilized in dermatology to enhance epidermal barrier function; it has been shown to induce the expression of PPARβ/δ and the lipid transporter ABCA12, promoting the delivery of fatty acids to lamellar granules in keratinocytes. It is also studied as a microbial metabolite that may influence metabolic health, including insulin resistance and obesity, through interactions with transcription factors like ZNF384.
