Drug intelligence / Profile preview
MEX-CD1 (Metal EXtraction - Chitosan DOTAGA 1) is a novel, high-molecular-weight (~300 kDa) superchelating polymer developed by MexBrain. It is composed of a reacetylated chitosan backbone functionalized with DOTAGA (1,4,7,10-tetraazacyclododecane-1-glutaric acid-4,7,10-triacetic acid) ligands. Designed as a dialysis-compatible extracorporeal medical device (Class III), MEX-CD1 is added directly to the dialysate during continuous veno-venous hemodialysis (CVVHD) rather than being administered systemically. Because of its large molecular size, it remains confined to the dialysate circuit and cannot cross the semipermeable dialysis membrane into the patient's bloodstream. It acts by capturing free, toxic metal cations (such as iron and copper) that diffuse across the membrane from the blood, preventing concentration equilibrium and enhancing metal extraction. MEX-CD1 is being clinically evaluated for the treatment of sepsis-associated acute kidney injury (AKI), Wilson's disease, acute-on-chronic liver failure (ACLF), and transfusion-dependent myelodysplastic syndromes (MDS) or myelofibrosis.
