MFE-23-his is a recombinant single-chain variable fragment (scFv) antibody directed against carcinoembryonic antigen (CEA). It was specifically developed for use in radioimmunoguided surgery (RIGS), a technique where a radiolabeled antibody is administered intravenously prior to surgery to help surgeons locate tumor margins and occult metastatic disease using a hand-held gamma-detecting probe. Due to its small molecular size (approximately 27 kDa), MFE-23-his exhibits rapid blood clearance and superior tumor penetration compared to full-length monoclonal antibodies, which reduces background noise and shortens the interval required between injection and surgery. The "his" suffix refers to a C-terminal hexahistidine tag incorporated to facilitate purification during production in bacterial systems. In clinical settings, it is typically radiolabeled with Iodine-125 (125I) for the detection of CEA-producing carcinomas, such as colorectal and pancreatic cancers.
