MFGM-RUTF is a specialized formulation of ready-to-use therapeutic food (RUTF) enhanced with milk fat globule membrane (MFGM), currently under clinical investigation for the treatment of children with severe acute malnutrition (SAM). Standard RUTF typically consists of peanut paste, sugar, vegetable oil, and skim milk powder; however, it lacks certain bioactive lipids and proteins found in mammalian milk that are critical for neurodevelopment. MFGM-RUTF incorporates an MFGM-containing whey protein/fat concentrate, providing essential sphingolipids (such as sphingomyelin), cholesterol, and phospholipids (including phosphatidylcholine and phosphatidylethanolamine). Additionally, because MFGM acts as a natural emulsifier, this formulation allows for the omission of synthetic hydrogenated vegetable oil emulsifiers, which may potentially improve gut barrier healing in malnourished children. The primary goals of MFGM-RUTF therapy are to improve neurodevelopmental recovery and reduce the incidence of poor clinical outcomes, such as death or hospitalization, compared to standard skim milk-based RUTF.
