mFLOX is a modified chemotherapy regimen consisting of bolus 5-fluorouracil (5-FU), folinic acid (leucovorin), and oxaliplatin. It is being evaluated by the Instituto do Cancer do Estado de São Paulo (ICESP) as a first-line treatment for patients with unresectable or metastatic pancreatic adenocarcinoma who are ineligible for the standard infusional FOLFIRINOX protocol due to factors such as poor performance status (ECOG 2) or hyperbilirubinemia. Oxaliplatin, a platinum-based antineoplastic agent, works by forming DNA crosslinks that inhibit replication and transcription. 5-fluorouracil is a pyrimidine analog that inhibits thymidylate synthase, while folinic acid is used to enhance the binding of 5-FU to its target. This specific bolus-based modification aims to provide a more tolerable alternative to intensive infusional regimens for patients with significant comorbidities or biliary obstruction.
