mFOLFIRINOX + anlotinib + sintilimab is a combination therapy regimen being evaluated for the first-line treatment of locally advanced or metastatic pancreatic cancer. The regimen combines the cytotoxic chemotherapy backbone mFOLFIRINOX (modified fluorouracil, leucovorin, irinotecan, and oxaliplatin) with anlotinib, a multi-targeted tyrosine kinase inhibitor, and sintilimab, an anti-PD-1 monoclonal antibody. Anlotinib suppresses tumor growth and angiogenesis by inhibiting VEGFR, FGFR, PDGFR, and c-Kit. Sintilimab acts as an immune checkpoint inhibitor, blocking the interaction between PD-1 and its ligands to restore T-cell mediated anti-tumor activity. This multi-modal approach, sponsored by Fujian Provincial Hospital, aims to enhance therapeutic efficacy by simultaneously targeting DNA replication, angiogenesis, and immune evasion in pancreatic ductal adenocarcinoma.
