mFOLFOX is a modified chemotherapy regimen consisting of oxaliplatin, leucovorin (folinic acid), and fluorouracil (5-FU). Oxaliplatin is a platinum-based antineoplastic agent that forms DNA cross-links, inhibiting DNA replication and transcription. Fluorouracil is a pyrimidine analog that acts as an antimetabolite, primarily by inhibiting thymidylate synthase to block DNA synthesis. Leucovorin is a reduced folate that is administered to stabilize the binding of fluorouracil to thymidylate synthase, thereby enhancing its cytotoxic effect.
