mFOLFOX + radiation therapy is a combination treatment regimen used primarily for certain gastrointestinal cancers such as rectal and esophageal cancer. The "mFOLFOX" component refers to a modified version of the FOLFOX chemotherapy protocol, which includes three drugs: - oxaliplatin (a platinum-based DNA crosslinker), - folinic acid (leucovorin/calcium folinate), which enhances the effect of fluorouracil, - fluorouracil (5-FU), an antimetabolite that inhibits thymidylate synthase and disrupts DNA synthesis. Radiation therapy uses high-energy x-rays to kill tumor cells and shrink tumors. When combined with mFOLFOX chemotherapy, this approach is called chemoradiotherapy or chemoradiation. This combination is used in both neoadjuvant (preoperative) and definitive settings for locally advanced rectal cancer and other gastrointestinal malignancies[1][2][4]. The mechanism involves direct cytotoxicity from chemotherapy agents plus local tumor control from radiotherapy.
