Miconazole + metronidazole is a fixed-dose combination therapy consisting of the imidazole antifungal miconazole and the nitroimidazole antibacterial and antiprotozoal agent metronidazole. Miconazole exerts its antifungal effect by inhibiting the enzyme lanosterol 14-alpha demethylase (CYP51), which is essential for the synthesis of ergosterol in fungal cell membranes; its depletion leads to membrane instability and cell death. Metronidazole is a prodrug that is reductively activated in anaerobic environments to form reactive intermediates that cause DNA strand breakage and inhibit nucleic acid synthesis in bacteria and protozoa. This combination is primarily indicated for the treatment of vaginal infections, including bacterial vaginosis (BV), vulvovaginal candidiasis, and trichomoniasis, as well as mixed vaginal infections. S.I.I.T. Srl is an Italian contract development and manufacturing organization (CDMO) that develops and manufactures this combination in various dosage forms, such as vaginal suppositories and tablets, for the global market.
