Micronized purified flavonoid fraction (MPFF) is a phlebotropic and vasoprotective pharmaceutical agent composed of 90% diosmin and 10% other flavonoids, primarily hesperidin. It is widely utilized for the management of chronic venous insufficiency (CVI) and hemorrhoidal disease. The micronization process reduces the particle size of the flavonoids to less than 2 micrometers, significantly enhancing gastrointestinal absorption and clinical efficacy compared to non-micronized formulations. MPFF exerts its therapeutic effects by increasing venous tone through the prolongation of norepinephrine's activity on the venous wall (via inhibition of catechol-O-methyltransferase), improving lymphatic drainage, and protecting the microcirculation. It also demonstrates potent anti-inflammatory properties by inhibiting leukocyte adhesion to the endothelium and reducing the expression of adhesion molecules such as ICAM-1 and VCAM-1, thereby preventing the release of inflammatory mediators and reducing capillary hyperpermeability.
