Overview

Microplegia is an undiluted blood cardioplegia technique used for myocardial protection during cardiac surgery, such as aortic or mitral valve replacement. Developed as an alternative to standard 4:1 diluted blood cardioplegia, microplegia utilizes the patient's own oxygenated blood as the primary vehicle, supplemented with concentrated additives like potassium chloride and magnesium sulfate. This approach is designed to stop the heart (cardioplegia) while providing optimal oxygen delivery and minimizing the systemic hemodilution associated with crystalloid-heavy solutions. Washington University School of Medicine has investigated this strategy in Phase 4 trials to determine its efficacy in reducing perioperative red blood cell transfusion requirements.
