Midazolam + hydroxyzine is an oral combination of two drugs used primarily for procedural sedation, especially in pediatric dentistry. Midazolam is a short-acting benzodiazepine with sedative, anxiolytic, amnesic, muscle relaxant, and anticonvulsant properties. It acts as a positive allosteric modulator at the gamma-aminobutyric acid type A (GABA-A) receptor in the central nervous system, enhancing inhibitory neurotransmission and producing sedation and anxiolysis[5]. Hydroxyzine is a first-generation antihistamine with sedative and antiemetic effects; it acts as an inverse agonist at the histamine H1 receptor and also has anticholinergic properties[6]. When combined, hydroxyzine can supplement or enhance the sedative effect of midazolam while reducing nausea or vomiting. This combination has been studied for use in children undergoing dental procedures to improve cooperation by providing deeper or longer-lasting sedation compared to either drug alone[1][2][3]. The safety profile appears favorable when used at appropriate doses under medical supervision.
