Midazolam + normal saline refers to a specific treatment arm in a randomized clinical trial conducted by the Institute of Liver and Biliary Sciences (ILBS), India, evaluating the prevention of postoperative nausea and vomiting (PONV). In this protocol, patients received 2 mg of midazolam intravenously 30 minutes before the end of laparoscopic surgery, along with normal saline. Midazolam is a short-acting benzodiazepine that functions as a positive allosteric modulator of GABA-A receptors. Beyond its standard use as an anxiolytic and sedative, it is hypothesized to exert antiemetic effects by modulating dopaminergic and GABAergic signaling in the central nervous system. The ILBS study compared this regimen to a standard combination of dexamethasone and ondansetron in high-risk patients.
