Overview

Midodrine is a small molecule, orally administered prodrug used primarily to treat symptomatic orthostatic hypotension (a sudden fall in blood pressure upon standing). After administration, midodrine is rapidly converted to its active metabolite, desglymidodrine, which selectively stimulates alpha-1 adrenergic receptors on arteriolar and venous vasculature. This leads to vasoconstriction and an increase in blood pressure. Desglymidodrine does not cross the blood-brain barrier significantly and thus has minimal central nervous system effects. The drug is also used off-label for conditions such as stress urinary incontinence and complications of liver cirrhosis (e.g., hepatorenal syndrome), often in combination with other agents like octreotide[2][3][5][6][7]. Common side effects include piloerection ("goosebumps"), scalp itching or tingling, chills, supine hypertension (high blood pressure when lying down), urinary retention or urgency, headache, flushing, abdominal pain, rash, and rarely bradycardia[3][6][8]. It should not be used in patients with severe organic heart disease, acute renal disease, urinary retention, pheochromocytoma or thyrotoxicosis[2].
