Milatuzumab-CL2A-SN-38 (also known as IMMU-115) is an experimental antibody-drug conjugate (ADC) developed by Immunomedics. It consists of milatuzumab, a humanized monoclonal antibody targeting the CD74 antigen, conjugated to SN-38, the active metabolite of the topoisomerase I inhibitor irinotecan. The conjugation utilizes a pH-sensitive, cleavable CL2A linker, which is designed to release the cytotoxic payload within the acidic environment of the lysosome or through slow hydrolysis in the circulation (with a serum half-life of approximately one day). CD74 is a cell-surface protein that is highly expressed and rapidly internalized in various hematological malignancies and solid tumors, including melanoma, pancreatic cancer, and B-cell lymphomas, making it an effective target for intracellular drug delivery. Preclinical studies have demonstrated significant antitumor activity in human tumor xenograft models of melanoma and other CD74-positive cancers.
