Overview

Milrinone + terlipressin is a combination of two pharmaceutical agents used together in specific clinical settings, particularly in patients with cardiac dysfunction and/or advanced liver disease. Milrinone is a phosphodiesterase-3 inhibitor that acts as an inotrope and vasodilator, increasing cardiac contractility while reducing systemic and pulmonary vascular resistance. Terlipressin is a synthetic vasopressin analog that acts primarily as a vasoconstrictor, especially within the splanchnic circulation. The combination is sometimes used to counteract the systemic hypotension induced by milrinone without significantly increasing pulmonary artery pressure or pulmonary vascular resistance. This approach has been studied in perioperative management of patients with pulmonary hypertension undergoing cardiac surgery, where terlipressin can help maintain mean arterial pressure after milrinone administration[2][3]. Terlipressin also has established roles in managing hepatorenal syndrome and acute variceal bleeding due to its potent splanchnic vasoconstrictive effects[5][6].
