Miltefosine + paromomycin is a combination therapy used for the treatment of visceral leishmaniasis, particularly in eastern Africa. This combination offers an alternative to the standard sodium stibogluconate plus paromomycin (SSG/PM) treatment. The combination provides similar efficacy to SSG/PM but with fewer injections, shorter treatment duration (14 days vs 17 days), and no risk of SSG-associated life-threatening cardiotoxicity, making it a more patient-friendly option for both children and adults. The combination also appears to reduce the risk of subsequent post-kala-azar dermal leishmaniasis (PKDL), which is important for reducing community transmission of visceral leishmaniasis.
