Mini-hyper-CVD is a dose-attenuated chemotherapy regimen developed at the University of Texas MD Anderson Cancer Center, primarily for the treatment of acute lymphoblastic leukemia (ALL). It is a modification of the intensive hyper-CVAD regimen, featuring a 50% dose reduction in cyclophosphamide and dexamethasone, the omission of anthracyclines (doxorubicin/Adriamycin), and the inclusion of vincristine. This regimen is designed to minimize toxicity while maintaining therapeutic efficacy, making it particularly suitable for older patients or for use as a backbone in combination with novel targeted therapies such as inotuzumab ozogamicin, ponatinib, or venetoclax. It typically serves as the 'A' cycle in an alternating treatment schedule that includes dose-reduced methotrexate and cytarabine.
