Minodronate + teriparatide is a combination therapy consisting of minodronate, a bisphosphonate antiresorptive agent, and teriparatide, an anabolic recombinant human parathyroid hormone analog (PTH 1-34). This combination is investigated for the treatment of osteoporosis in patients at high risk of fracture. Minodronate works by inhibiting osteoclast-mediated bone resorption, thereby reducing bone turnover and loss. Teriparatide acts as an agonist at the parathyroid hormone 1 receptor to stimulate osteoblastic activity and promote new bone formation. The rationale for combining these agents is to potentially expand the "anabolic window"—the period during which bone formation exceeds resorption—by simultaneously suppressing excessive resorption (via minodronate) while stimulating new bone growth (via teriparatide). While sequential or combined use of bisphosphonates with PTH analogs has been studied with other bisphosphonates such as alendronate or risedronate, data on minodronate specifically in combination with teriparatide are limited but follow similar mechanistic principles[1][5].
