Minoxidil + N-acetyl cysteine is a combination therapy used primarily for the treatment of androgenetic alopecia (AGA) in men. This combination leverages complementary mechanisms to address hair loss: 1. **Minoxidil** works as a vasodilator that opens ATP-sensitive potassium channels in vascular smooth muscle cells, improving blood flow to hair follicles. It also promotes the survival of dermal papillary cells by activating extracellular signal-regulated kinase (ERK) and Akt pathways, preventing cell death by increasing the BCl-2/Bax ratio, and prolonging the anagen (growth) phase of hair follicles[8]. 2. **N-acetyl cysteine (NAC)** functions as a reactive oxygen species (ROS) scavenger that addresses the oxidative stress component in AGA pathogenesis. NAC replenishes glutathione levels and provides antioxidant protection to hair follicles. It has been shown to improve trichoscopic features of AGA, including increasing terminal hair count and decreasing vellus hair count[2][7]. Clinical studies have demonstrated that the combination of topical minoxidil 5% twice daily with oral NAC 600 mg three times daily for 4 months can significantly improve hair growth parameters compared to either treatment alone or no treatment[1][2].
