miR-146a-EVs are extracellular vesicles (EVs), such as exosomes, engineered or enriched to contain high levels of the microRNA miR-146a-5p. These vesicles are typically generated by transfecting human umbilical cord mesenchymal stem cells (hucMSCs) or other cell types to overexpress miR-146a-5p, after which the secreted EVs are isolated and purified. miR-146a-EVs act as a form of RNA therapy and cell therapy, transferring miR-146a-5p to recipient cells and tissues where they exert anti-inflammatory effects. Mechanistically, miR-146a-5p targets and suppresses the expression of TNF receptor-associated factor 6 (TRAF6) and interleukin-1 receptor-associated kinase 1 (IRAK1), thus inhibiting downstream NF-κB and NLRP3 inflammasome activation, reducing proinflammatory cytokine release, and promoting resolution of inflammation. The approach has been studied primarily for inflammatory and immune-mediated diseases such as acute lung injury, airway inflammation, and potentially neuroinflammation, often using intravenous or nebulized delivery[2][4][6].
